Analisis Desain Formulir Informed Consent Di Klinik Laras Hati Sewon Bantul

Abstract

Background: The form containing the informed consent form is called the informed consent form. Based on the
initial survey conducted, the Laras Hati clinic informed consent form is separated into two forms, namely approval
or refusal for medical treatment is simplified by combining it into one form.
Objective: Analyzing the design of the informed consent form covering the anatomical, physical, content and
redesigning aspects of the Laras Hati Clinic informed consent form.
Method: Qualitative descriptive research, analyzing the design of the form from the anatomical, physical and
content aspects of the informed consent form.
Result: Design analysis is based on anatomical aspects that are not in accordance with form design methods in
headings and form instructions. The physical aspect of the form number and time of year printed or published is
not contained in the form. Content aspects that do not match the rules on patient identity data and clinical data.
Conclusion: Analysis of the anatomical, physical and content aspects there are still data items that are not stated
in the form. The shortcomings of these items were completed in the informed consent form on the redesign.
