Background: a good preparedness in birth and postpartum phase is the key for women and her babies. It needed to prevent complication, risk and maternal and neonatal deaths. In Bantul District, there were a fluctuate rate for maternal mortality rate in 6 year later. A sharp increased due on 2019 (13 cases / 99.45 per 100.000 live births) to 2020 (20 cases/157.6 per 100.000 live births). It should be prevented by synergistic action which involved midwife, women and her family and community. Method: educational approach by a discussion between midwife, 2 cadres and 8 pregnant women in Gabusan Timbulharjo Village. This discussion used “Buku KIA”, a personal heath book for pregnant woman and her baby, and colored card to measure women’s preparedness in birth and postpartum phase. Result: Women’s preparedness in birth and postpartum phase was sufficient. All participant played an active responses through the discussion. Psychology aspect during birth was the most popular topic in this discussion. However, both cadre and the women have limited information about postpartum care. Conclusion: women feel more ready in birth and postpartum preparedness, while cadre more understand and ready to assist people in birth and postpartum preparedness. This simple partnership might be developed for future action in maternal and neonatal health. “Buku KIA”, a personal heath book for pregnant woman and her baby, was useful tool in this educational approach
